How to use Your Order Form

Your Order Form is an order form generator designed to make the ordering process easier for prescribers at Bayview Pharmacy. With this tool, you can add formulas from the Formulas tab and have them automatically populate on the order form. In this article, we'll show you how to use Your Order Form to streamline your ordering process and save time.

Step 1: Accessing Your Order Form

To access Your Order Form, log in to your account on Bayview Pharmacy's website. From there, click on Your Order Form to open the order form generator.

Step 2: Adding Formulas

Once you're in Your Order Form, navigate to the Formulas tab.

Here, you can search through our database of thousands of pre-populated formulas that are available.

Once you find the formula(s) you want to prescribe, simply click the Add to Order Form button.

They will automatically be added to Your Order Form.

Step 3: Inputting Patient Information

Next, navigate back to Your Order Form and you will need to input patient information at the top of the order form. This includes patient first and last name, address, date of birth, sex, email, and pick-up/shipping preference.

Step 4: Reviewing and Submitting the Order

Once you have selected the desired formula you want to prescribe for the patient, click the Print Order Form button at the bottom of Your Order Form.

You may need to scale the order form accordingly to fit the printable area of the paper. Typically, we suggest to scale the order form to 75-80%.
