It is a compounded, prescription-only preparation and is not FDA-approved. Standard first-line therapy for hypothyroidism is commercial levothyroxine; compounding is used when a custom dose or ratio is needed, or to avoid specific dyes, fillers, or allergens.

What is Compounded Thyroid (T3/T4) Capsules?

Compounded thyroid is a prescription preparation that replaces the thyroid hormone an underactive thyroid does not make in adequate amounts. Bayview compounds combination liothyronine/levothyroxine (T3/T4) capsules in custom ratios, liothyronine (T3)-only capsules, and desiccated (natural) thyroid capsules.

Each preparation is made to the strength and ratio your prescriber specifies, which allows fine dose adjustments and formulations without certain dyes, fillers, or allergens.

Compounded thyroid is not an FDA-approved product. Commercial levothyroxine is the standard first-line treatment for hypothyroidism. Compounded thyroid is prepared by Bayview only with a valid prescription, when your provider decides that a custom dose or ratio, or the need to avoid specific ingredients, makes it appropriate.

How does it work?

Thyroid hormone regulates metabolism, energy use, heart rate, and body temperature. Levothyroxine is synthetic T4 and liothyronine is synthetic T3; both replace the hormones an underactive thyroid does not produce in sufficient amounts (StatPearls: Thyroid Hormone Physiology; StatPearls: Levothyroxine).

What is it used for?

Thyroid hormone replacement treats hypothyroidism (an underactive thyroid). It is not a treatment for weight loss. Whether a compounded preparation rather than a commercial product is appropriate is a decision your provider makes based on your labs and history (MedlinePlus: Levothyroxine).

T4, T3, and desiccated thyroid, what's the difference?

Levothyroxine (T4) is the storage form the body steadily converts into active hormone, giving stable, long-acting levels. Liothyronine (T3) is the active hormone; it acts faster and clears sooner. Desiccated (natural) thyroid is derived from porcine thyroid and contains both T4 and T3 (MedlinePlus: Liothyronine).

How is it used?

Thyroid capsules are usually taken by mouth once daily on an empty stomach, often 30 to 60 minutes before breakfast. Your provider sets the dose and adjusts it using TSH and other thyroid labs. Separate calcium, iron, and antacids by several hours, since they reduce absorption.

Side effects & safety

Taking more thyroid hormone than the body needs can cause palpitations, tremor, insomnia, and heat intolerance. Report symptoms of over- or under-treatment so your dose can be adjusted (MedlinePlus: Levothyroxine).

Who should not use it?

Use requires caution in people with heart disease, arrhythmia, or adrenal insufficiency, and in older adults, who may need lower starting doses. Tell your provider about all of your conditions and medicines before starting.

How should it be stored?

Store at room temperature, away from heat, moisture, and light, and keep out of reach of children. Use by the beyond-use date on the label.

Why is this compounded?

Compounding lets Bayview prepare a specific T3/T4 ratio or custom strength, a T3-only or desiccated option, or a capsule free of particular dyes or fillers when a commercial product does not meet a patient's needs. Clinical details for prescribers

Regulatory status. Thyroid hormones are not controlled substances. Levothyroxine (T4) and liothyronine (T3) are FDA-approved as single-agent products; custom-ratio T3/T4 combination capsules and compounded desiccated thyroid capsules are not FDA-approved and are prepared per prescription by a 503A pharmacy.

Formulation & dosing. Compounded as T3/T4 capsules in custom mcg ratios (e.g., 8/25, 20/95, 25/90), liothyronine-only capsules (4 to 10 mcg), and desiccated thyroid capsules (5 to 65 mg). Commonly dosed once daily on an empty stomach, 30 to 60 minutes before breakfast, separated by 4 hours from calcium, iron, and antacids. Recheck TSH (± free T4/T3) 6 to 8 weeks after any dose change.

Counseling / clinical points:

  • Narrow therapeutic index, counsel on consistent timing and consistent product use
  • Use caution with T3-containing regimens in elderly patients and those with cardiovascular disease; T3 is substantially more potent than T4
  • Review for symptoms of over-replacement: palpitations, tremor, heat intolerance, insomnia, unintended weight loss
  • Guidelines favor levothyroxine monotherapy for most patients, combination therapy is an individualized decision for selected patients

How is my dose adjusted?

Your provider individualizes the dose and checks TSH and other thyroid labs over time. Report signs of too much hormone, palpitations, tremor, insomnia, or heat intolerance, so the dose can be adjusted.

Are compounded medications FDA-approved?

Compounded medications are not FDA-approved. They're prepared by a licensed pharmacy pursuant to a valid prescription for an individual patient, under USP standards. The FDA does not review compounded medications for safety or effectiveness.

Can you fill a prescription from my own provider?

Yes. Your provider can send your prescription to Bayview Pharmacy by e-prescribe, fax, or phone, or you can transfer an existing prescription from another pharmacy.

Do I need a prescription?

Yes. All of our medications require a valid prescription from a licensed provider. We're a dispensing and compounding pharmacy, so we don't provide consultations or write prescriptions. If you don't have one yet, you'll need to find and see a provider first.

How does shipping work?

Once your prescription is verified, we prepare your medication and ship it discreetly in plain packaging either via UPS or USPS. A pharmacy technician confirms the details with you before it goes out, and we're available if you have any questions.

How do providers send a prescription to Bayview?

Providers can submit a prescription three ways:

  • E-prescribe: Search Bayview Pharmacy, Warwick, RI (NCPDP 4106882) in your EHR. For compounds, use the compound, custom, or free-text option and include the full formulation, strength, quantity, refills, and directions.
  • Fax: Send a signed order to 401-284-4506 (alt. 401-210-2757) with patient and prescriber details, medication, quantity, refills, directions, signature, and date.
  • Phone: Call 401-284-4505 to submit, clarify, or coordinate refills.

A valid prescription is required, and compounded medications are not FDA-approved.

How do refills and automatic refills work?

Refills are coordinated with your prescriber and your prescription. You can request a refill through our website or by calling us, and we'll handle the rest as long as you have refills remaining on a valid prescription. You can also opt into automatic refills on non-controlled substances, with no need to worry about running out of your medication. We can either ship it to you automatically, or ready for pickup.

What is compounding?

Compounding is the practice of preparing a medication tailored to an individual patient's prescription. Instead of a mass-produced product, a compounding pharmacy makes the medication to the specific strength, form, or combination your provider orders.
