Step 2: Fill in Patient Information
On the New Prescription Order Form - Semaglutide order form, fill in your patient information section, making sure that all of the information is up-to-date and a correct phone number so we can contact the patient about their treatment plan.
Step 3: Fill in Prescriber Information
After filling out the patient information, fill out your prescriber information in the section below it.
Step 4: Select Dose/Formula to Prescribe
For new patients, we recommend starting with the target dose of 0.25mg, since the half-life of Semaglutide is 5-7 days and we are trying to minimize side effects.
If a dose is ineffective, increase the dosage by 0.25mg to the next stage until an effective dose is found.

As you can see, you begin by prescribing 0.5 mg/mL Sublingual Semaglutide 0.5 mL qw x 4w, and if not effective, increase dose to 1 mL qw x 4w, and so on.
If there are no side effects and that dose is effective, continue with that strength and dose.
