Spironolactone is a potent potassium-sparing diuretic and aldosterone antagonist commonly used in compounded formulations to address conditions such as hypertension, heart failure, and certain cases of.
Spironolactone is a potassium-sparing diuretic and aldosterone antagonist. It acts as a mineralocorticoid receptor antagonist, blocking aldosterone so that sodium reabsorption is reduced while potassium is retained.[1] FDA-approved spironolactone tablets are indicated for heart failure with reduced ejection fraction, hypertension (as add-on therapy), edema due to cirrhosis or nephrotic syndrome, and primary hyperaldosteronism.[2]
Spironolactone also has antiandrogen effects. Off-label, non-FDA-approved uses include acne vulgaris and hirsutism.[1] It is used off-label as an alternative agent for persistent or late-onset acne in adult women, where it may reduce sebum production by blocking dihydrotestosterone binding to androgen receptors, and it is used in select patients with hirsutism, although high-level evidence for these uses is lacking.[1]
Spironolactone tablets are taken by mouth, and food increases the bioavailability of spironolactone.[2] It is rapidly and extensively metabolized to active metabolites, including canrenone and 7-alpha-thiomethylspironolactone, whose half-lives (about 13.8 to 16.5 hours) are much longer than that of the parent drug (about 1.4 hours).[2] Labeled dosing for hypertension is given in single or divided daily doses.[2] Prescribers should be aware of potential side effects, including hyperkalemia, gynecomastia, and gastrointestinal effects such as nausea, vomiting, and diarrhea, and should monitor serum potassium and renal function.[2][1]
