A small arrow pointing down

This formulation is a compounded topical gel containing Baclofen (6%), Clonidine HCl (0.2%), Diclofenac Sodium (3%), Gabapentin (6%), and Lidocaine (2.5%). It is a semisolid formulation that is dispensed through a pump mechanism, allowing for easy and precise application on the skin. The gel-like consistency ensures targeted treatment or absorption of the active ingredients.

Baclofen is a muscle relaxant and antispastic agent that works by calming the nerves and reducing pain. Clonidine HCl is an alpha-2 adrenergic agonist that can help manage neuropathic pain by blocking pain signals to the brain. Diclofenac Sodium is a nonsteroidal anti-inflammatory drug (NSAID) that reduces inflammation and pain in the body. Gabapentin is an anticonvulsant and nerve pain medication that is often used to treat seizures and pain caused by shingles. Lidocaine is a local anesthetic that works by causing temporary numbness/loss of feeling in the skin and mucous membranes.

This combination of medications in a topical gel form is designed to provide targeted relief for a variety of conditions. It can be used to manage pain associated with conditions such as neuropathy, fibromyalgia, and other types of chronic pain. The gel formulation allows for direct application to the affected area, reducing systemic side effects often associated with oral medications.

It is important to use this medication as directed by your healthcare provider. Apply a thin layer of the gel to the affected area, usually 3 to 4 times daily or as directed by your doctor. Wash your hands immediately after use unless you are treating an area on the hands. Avoid contact with the eyes, nose, mouth, and any areas of broken skin.

As with any medication, this formulation may cause side effects. These can include skin irritation at the application site, dizziness, dry mouth, or constipation. If any of these effects persist or worsen, notify your doctor or pharmacist promptly.

Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects. However, get medical help right away if you notice any symptoms of a serious allergic reaction, including rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, or trouble breathing.

This is not a complete list of possible side effects. If you notice other effects not listed above, contact your doctor or pharmacist. Drug Interactions

A small arrow pointing down

This compounded formulation contains several active ingredients: Baclofen, Clonidine HCl, Diclofenac Sodium, Gabapentin, and Lidocaine. Each of these medications can interact with other drugs, potentially altering their effects. It's important to inform your healthcare provider of all medications you are currently taking, including prescription drugs, over-the-counter medications, and dietary supplements.

Baclofen, a muscle relaxant and antispastic agent, may interact with other central nervous system depressants, including alcohol, antihistamines, sedatives, and opioids, potentially enhancing their effects and increasing the risk of respiratory depression, sedation, and low blood pressure. Concurrent use with monoamine oxidase inhibitors (MAOIs) may result in increased baclofen blood levels and effects.

Clonidine HCl, an alpha-2 adrenergic agonist, can interact with beta-blockers, calcium channel blockers, and digitalis, potentially causing bradycardia and hypotension. Concurrent use with tricyclic antidepressants may reduce clonidine's effectiveness. Abrupt discontinuation of clonidine while taking beta-blockers may increase the risk of rebound hypertension.

Diclofenac Sodium, a nonsteroidal anti-inflammatory drug (NSAID), may interact with anticoagulants, antiplatelet drugs, and other NSAIDs, increasing the risk of bleeding. Concurrent use with angiotensin-converting enzyme (ACE) inhibitors, angiotensin receptor blockers (ARBs), or diuretics may impair renal function. Diclofenac may also reduce the effectiveness of antihypertensive drugs.

Gabapentin, an anticonvulsant, may interact with antacids, reducing gabapentin's absorption. Concurrent use with central nervous system depressants may enhance their effects. Gabapentin may also increase the risk of suicidal thoughts or behavior when used with other antiepileptic drugs.

Lidocaine, a local anesthetic, may interact with other local anesthetics and antiarrhythmic drugs, potentially causing additive systemic toxicity. Concurrent use with monoamine oxidase inhibitors (MAOIs) or tricyclic antidepressants may increase the risk of severe, potentially fatal, cardiac arrhythmias.

This formulation is designed for topical application, which may reduce the risk of systemic drug interactions. However, systemic absorption can still occur, especially when applied over large skin areas or under occlusive dressings. Therefore, it's important to use this medication as directed by your healthcare provider.

This compounded formulation is used for the management of neuropathic pain, muscle spasms, and inflammation. Baclofen and Gabapentin are used for their antispastic and anticonvulsant properties, respectively, which can help manage neuropathic pain. Clonidine HCl can provide analgesia and sedation. Diclofenac Sodium is an NSAID that can reduce inflammation and pain. Lidocaine is a local anesthetic that provides immediate pain relief by numbing the skin.

If you have any questions about this formulation or its potential drug interactions, please reach out to us at Bayview Pharmacy. We are a team of knowledgeable compounding pharmacists, and we are here to help.

How Does it Work

A small arrow pointing down

This topical gel formulation combines five active ingredients: Baclofen, Clonidine HCl, Diclofenac Sodium, Gabapentin, and Lidocaine. Each of these ingredients plays a unique role in managing pain and inflammation, and their combined effect can provide significant relief for a variety of disease states.

Baclofen is a muscle relaxant and antispastic agent. It works by inhibiting the transmission of reflexes at the spinal level, which can help to reduce muscle spasms, pain, and stiffness. This makes it particularly useful in conditions such as multiple sclerosis and spinal cord injuries.

Clonidine HCl is an alpha-2 adrenergic agonist. It works by stimulating receptors in the brain that reduce the transmission of pain signals through the nervous system. This can help to alleviate neuropathic pain and other types of chronic pain.

Diclofenac Sodium is a nonsteroidal anti-inflammatory drug (NSAID). It works by reducing the production of prostaglandins, chemicals in the body that cause inflammation and increase pain sensitivity. This makes it effective in treating conditions such as osteoarthritis and rheumatoid arthritis.

Gabapentin is an anticonvulsant and nerve pain medication. It works by stabilizing electrical activity in the nervous system and reducing the release of neurotransmitters that transmit pain signals. This can help to alleviate neuropathic pain, such as that caused by shingles or diabetic neuropathy.

Lidocaine is a local anesthetic. It works by blocking sodium channels in the nerve cells, preventing the initiation and conduction of nerve impulses. This can provide immediate, localized relief from pain and discomfort.

The gel formulation allows for targeted application of these active ingredients directly to the area of pain or inflammation. The semisolid consistency and pump mechanism facilitate easy and precise application, while the topical route of administration minimizes systemic absorption and potential side effects.

This formulation can be used to treat a variety of disease states that involve pain and inflammation, including but not limited to neuropathic pain, musculoskeletal pain, osteoarthritis, rheumatoid arthritis, multiple sclerosis, and spinal cord injuries. As with any medication, it should be used under the supervision of a healthcare provider, and patients should follow the prescribed dosage and application instructions to ensure safe and effective use.

Other Uses

A small arrow pointing down

This formulation is primarily intended for treating neuropathic pain, inflammation and certain muscle conditions. However, it has other uses too. They are as follows:

Baclofen in this formulation can act as a muscle relaxant and an antispastic agent. Therefore, apart from managing symptoms of muscle spasms, stiffness and pain, it may be useful for conditions like multiple sclerosis or spinal cord injury. Clonidine treats high blood pressure; hence it may help manage withdrawal symptoms from opioids or smoking cessation. It also has potential applications in addressing attention deficit hyperactivity disorder (ADHD).

Diclofenac Sodium, a potent nonsteroidal anti-inflammatory drug (NSAID), is effective in treating conditions other than inflammation and pain, such as migraines, period cramps, or certain types of arthritis. Its anti-inflammatory action could be helpful in reducing swelling and pain associated with these conditions.

Gabapentin is primarily used to control seizures and to treat restless legs syndrome (RLS). Additional applications include its use for certain types of nerve pain. It is also sometimes prescribed to manage alcohol withdrawal, and certain types of mood disorders and anxiety — although these are considered off-label usages.

Lidocaine serves as a local anesthetic and ant arrhythmic agent in various medical procedures. Therefore, apart from providing relief from pain and itching caused by certain skin conditions such as scrapes, burns, insect bites, and hemorrhoids, Lidocaine is often utilized for temporary relief of pain from minor mouth problems. Outside of our formulation, Lidocaine may also be used in minor surgical procedures to numb the surgical area.

It is crucial to remember that this formulation is for topical use only and should not be ingested, inhaled, or applied into eyes, nose, or mouth. Patients must also consult with their healthcare provider before using this formulation, especially if they have conditions like cardiovascular disease, liver disease, kidney disease or a history of drug addiction. Always use this medication under the supervision of a healthcare provider and as directed by your physician.
